We are seeking to expand our team of Support Coordinators.
We are looking for an experienced Support Coordinator to join our team who can immediately take on a full case load of participants and implement their plans in accordance with their goals and desired outcomes. We are looking for someone who has a person-centred approach when working with participants and their support network, to establish a Support Plan, facilitating choice and control in accordance with the principles of the NDIS, and implementing all supports within the participant’s plan including formal, mainstream and funded supports.
